1. I declare that to the best of my knowledge my fish entered in this show are not KHV survivors,
have not been in contact with any fish known to have KHV or be a survivor of KHV or treated
with KHV vaccine.
2. The registrant agrees to indemnify all operators, associates, or sponsors (including members
and officers of these organizations) (hereafter collective known as the SHOW) from all legal
harm. In addition, registrant agrees to pay all legal fees, court costs, and damages that may arise
from registrant taking any civil action against the SHOW due to or caused as a result of any
action or negligence of any party.
BY SIGNING BELOW, I AGREE TO ALL THE TERMS AND CONDITIONS OF THIS
INDEMNIFICATION AND THE PRINTED KOI SHOW RULES AND REGULATIONS.
